Your role in making a referral could be not only life changing, but life saving for someone facing vision loss.

For referral information, you have three options:

1. Complete the form on this page.
2. Contact the Center in San Diego (1.619.583.1542) or in Vista (1.760.758.5956).
3. Contact us at referral@sdcb.org.

We welcome all referrals to the San Diego Center for the Blind (SDCB) for our services. Here are some of the kinds of referrals we receive:

• Adult children referring senior parents with failing eye sight.
• Neighbors or friends concerned about someone with whom they are close.
• Spouses of baby boomers concerned about their loved one's vision.
• And other people that are simply concerned about someone who may be experiencing vision loss.
  

How To Refer

To refer a someone, please fill out the referral for services and submit online or fax to SDCB at 619.583.2335. Center staff will contact the individual. (Attention doctors and medical personnel: please submit patient or client referrals via Medical-Professional-Referrals).
